The Disease Prevention and Care Coordinator will work within the Bureau of Communicable Diseases, Harm Reduction Unit, Harm Reduction Section to provide subject matter expertise, technical assistance, and outreach support on hepatitis C, HIV, and STIs. This position will work closely with the Harm Reduction Response Team to provide rapid and confirmatory hepatitis C and HIV testing to participants engaged in harm reduction services, educate participants about disease processes and transmission, and will work to present this information through a harm reduction lens. The Disease Prevention and Care Coordinator will assist HRRT participants in care navigation and be knowledgeable about medical resources throughout Wisconsin.

The Disease Prevention and Care Coordinator will supplement ongoing communicable disease surveillance and intervention efforts within the Bureau of Communicable Diseases that focus on priority populations. These included justice-involved persons, pregnant persons, infants who have been perinatally exposed to communicable diseases, and persons living with co-diagnoses of HIV, hepatitis C, and/or syphilis. Justice-involved persons are a population of focus for Wisconsin's hepatitis elimination planning, and the Disease Prevention and Care Coordinator will be expected to continue outreach work with jails across the state to provide hepatitis C testing.

This position will also have the unique opportunity to provide technical assistance to harm reduction agencies across Wisconsin in regard to service development and expansion, rapid HCV and HIV testing integration, and program development. This role will also provide technical assistance to multiple teams within the Harm Reduction Section on an ad hoc basis. This role will work closely with the Viral Hepatitis Program Coordinator, the Hepatitis C Epidemiologist, and the Hepatitis C Surveillance Specialist within BCD and HIV Surveillance Teams to remain up-to-date on the latest disease trends in Wisconsin.

Duties include:
* Serve as a subject matter expert in disease education, prevention, and counseling for the Harm Reduction Response Team.
* Assist with care navigation services for HRRT clients with a diagnosis of hepatitis C, HIV, or STIs.
* Collaborate with the HRRT Medical Director to provide direct medical care to HRRT participants as appropriate for scope of practice, and direct clients to more advanced services when needed.
* Work with local and state partners to identify resource gaps within harm reduction programs across Wisconsin and supplement existing services.
* Work with county jails and their health systems across Wisconsin to implement hepatitis C testing and treatment programs.
* Protocol and program development, technical assistance to outside agencies, progress reports, and presentations as needed.
* Provide ongoing support to the Viral Hepatitis Program Coordinator, Hepatitis C Epidemiologist, and Hepatitis C Surveillance Specialist within BCD as well as members of the HIV Surveillance Team.
* Specific focus on people with co-diagnoses of HIV, hepatitis C, and/or syphilis and perinatal cases of hepatitis C.

• 10% Identifies, promotes, and maintains external partnerships to support the outreach program
• 15% Schedules and secures resources and communicates logistics in support of an outreach program
• 40% Delivers outreach program content and materials to community members
• 15% Researches, develops, and facilitates outreach program content and materials
• 10% Develops and evaluates curriculum and programs and provides recommendations for improvement
• 5% Leads the implementation of programs, policies, and procedures through oversight of day-to-day activities for program staff and student and community volunteers
• 5% Providing rapid and confirmatory testing for hepatitis C, HIV, and STIs - including through blood draws.

Relevant areas of specialization include, but are not limited to, public health, infectious disease monitoring/management, social work, epidemiology, social sciences.

Required:
* Experience working with communicable diseases like hepatitis C, HIV, and STIs including providing counseling, testing, and referral services.
* Experience navigating medical systems to refer clients to care and treatment services for hepatitis C, HIV, and STI diagnoses.

Preferred:
* Experience with phlebotomy, specimen transportation, and laboratory operations.
* Experience working with syringe service programs, service delivery/mobile outreach, familiarity with drug use trends, and safer injection and use techniques.
* Experience facilitating program development and expansion across local, county, state jurisdictions.
* Experience working with local and tribal health departments.
* Experience with disease surveillance and reporting, biostatistics, epidemiology, and social determinants of health.
* Experience coalition building and creating community partnerships.
* Experience with emergency medicine, field aid/basic first aid.

Minimum $49,000 ANNUAL (12 months)

The starting salary for the position is $49,000 but is negotiable based on experience and qualifications.

This position will be located at the Rhinelander State office building in Rhinelander, WI.

This position has been identified as a position of trust with access to vulnerable populations. The selected candidate will be required to pass an initial caregiver check to be eligible for employment under the Wisconsin Caregiver Law and every four years.
